For a contingent worker, the forms under welcome activity include - Electronic Disclosure and Biographic Information. These forms appear only when the contingent worker has completed the Self Registration and signs in for the first time.
Forms under welcome activity of a contingent worker:
After "Welcome" page, the contingent worker must complete the following two forms -
Electronic Disclosure
Select the Consent checkbox and then click the Electronically Sign button.

Biographic Information
Fill out the required fields as indicated in the form below. The employee personal information entered in the invitation will be auto populated in Biographic Information such as First Name, Last Name, Phone Number, Email, and Home Address.

Note: If "showNameOnCWRBI" is set to "OFF", the name and preferred name information will not show on EBI page irrespective of "EnablePreferredNameonEBI" property value in Product Preferences. Please refer to the following section for more details on "EnablePreferredNameonEBI" property.

Also, both "CellPhoneOptional" and "HomePhoneOptional" properties cannot be changed to "ON" in product preferences. The following error message triggers if both are turned on at a time.
