From On the Insert Form Field popup page, select the Desired Input Type and Form Field that form field you wish to attach to the Smart Form. You can choose to make this Form Field form field mandatory as well as change the label that will be attached to the input type. Click OKAfter entering the details, click "OK".

To create an Attribute Revision, click the Menu icon and click Create Revision under Revisions.

Step 5 - Edit Supplemental Attribute Data
From a Attribute Revision, Administrative Users are able to add new attributes and modify existing attributes.
Your current attributes can be seen on On the Attribute Revision page. Select the Attribute Type , select the attribute you wish to configure, and you will see all attributes of that type displayed in the drop down.

When the Administrative User selects the an attribute (in this case Research), they are presented with a row of icons that provide them functionality specific to that single attribute. From this page When clicked on this link, we can see the Smart Form that we edited in our Content Revision with the appropriate form field and input typewith the edit box to input the attribute value.

If you wish want to add a different data value to another Company type, you can do so by repeating the process described above process. Say for instanceFor example, if you want to add a different Company Contact to the Company type Retail.Select for the "Retail" company type, select "Retail" under the Company attribute type and click on the the attribute"Manage Attributes".
Click on the "Add/View Supplemental Attributes" link on the Manage Attribute page.
Enter a value for the Company Contact and click Savesave the entry.
Now that you have defined two different values defined for two different types of Companies. Back on the Revision Attributes page, click Save, return to the Manage Attribute Revision page and click "Save".
