The following are the states with State W-4 Forms. Among them, the four states Colorado (CO), New Mexico (NM), North Dakota (ND), Utah (UT) use Federal Form W-4 for their state tax purposes. Depending on where the employee works or lives, the corresponding state tax W-4 form appears in the New Hire's "Work Location Tax Withholding" or "Residence Location Tax Withholding" activities.
