The Office of Federal Contractor Compliance Programs (OFCCP) has released an updated version of the Voluntary Self-Identification of Disability Form, valid until April 30, 2026. As part of this update, the new form has been implemented in Smart Onboarding and will be included in the New Hire's onboarding process starting June 1, 2023.

How do you access the new version of Voluntary Self-Identification of Disability form?

The new version of the Voluntary Self-Identification of Disability Form is effective for New Hires with a start date on or after June 1, 2023. This updated form will be accessible to New Hires under "New Hire Forms" in the onboarding process.

How do you identify the new version of the form?

Apart from the content/text changes, the new version of Voluntary Self-Identification Disability Form will have a version expiry date of Apr 30, 2026 displayed on the top right corner.

How do you complete and download the signed form?

Read the form, select one option that applies to you, and then click the "Electronically Sign" button.

The completed form can be downloaded by clicking the "Download Signed Forms" icon located in the bottom right corner of the page.

What happens to the invitations that have the activity routed but not completed before the new version effective date?

In this case, the form version depends on the start date of the New Hire's invitation.

• Any invitation with a start date on or after the effective date of the new version (June 1, 2023) that has been routed to the activity but not completed will display the new version of the form.

  For example, an invitation with a start date of June 2, 2023, will show the new version of the form.

  
  

  
  

• If the start date is before the new version effective date (Jun 01, 2023), the previous version of the form appears in the New Hire's process. 

Which form do you get if the process is restarted after the new version effective date?

For transactions that were completed before the new version effective date and restarted afterward, the version of the "Voluntary Self-Identification of Disability" form displayed depends on the start date of the invitation. If the start date is on or after June 1, 2023, the new version of the form will be shown whereas if the start date is before June 1, 2023, the previous version of the form will be displayed when the activity is accessed by the New Hire.
