US State Tax Forms

The following US states have their state withholding forms. Among them, New Mexico (NM), North Dakota (ND), and Utah (UT) use the Federal W-4 for their state tax withholding. Prior to 2022, Colorado also utilized the Federal W-4 for state tax withholding. However, starting in 2022, a new Colorado Employee Withholding Certificate (Form DR 0004) has been introduced for state tax purposes. Employees can choose to complete either the new Colorado Employee Withholding Certificate (Form DR 0004) or continue using the Federal W-4 for their state tax withholding. For more details on completing Colorado DR 0004 Form, please click here.

Depending on where the employee works or lives, the corresponding state tax form appears in the New Hire's "Work Location Tax Withholding" or "Residence Location Tax Withholding" activities.
